Symptom narrative

Following vaccine administration, pt sitting partially upright on exam table, leaning against the wall with her eyes closed. Pt skin was pale. Assisted pt to lying position on exam table.Pt became stiff/rigid and skin became pink. Pt opened her eyes and became responsive and alert to person, place within seconds. Pt was given juice and crackers and ate/drank without difficulty. Pt stood from seated position and was observed for 2-3 minutes prior to discharge. Pt denied complaints and ambulated without difficulty. Pt mother remained with pt during entire visit.
