Symptom narrative

Patient came to pharmacy 6/22/26 and reported that she now has shingles and is being treated with antibiotics and antiviral medication. I informed the patient that the vaccine does not cause condition. However, she wanted to continue report of adverse reaction. Her shingles outbreak is contained to the scalp area and has been seen by a provider and has been prescribed both antibiotic and antiviral to treat. I counseled the patient to continue therapy and follow up with provider if condition worsens.
