At 1038 was notified by MA that patient that patient was not feeling well after vaccine administration of Bexsero at 1036. Arrived in exam room at 1039, patient was sitting down on exam table alert and oriented. Patient stated he was feeling dizzy and so we laid him on exam table. Patient?s eyes rolled back and he became stiff. Code Yellow was called. Provider, APRN arrived in room at 1040 and assessed patient. No loss of consciousness. At 1042 vitals HR=68, BP=126/76, R=15, O2sat=98. At 1045, primary provider, MD arrived in room and assessed patient. Patient notified us that he soiled (urine) himself during seizure. Instructed to wait in exam room for 20 min. At 1106 vitals HR=84, BP 130/91, R=18, O2sat=98 then patient ambulated to restroom. Vitals taken again before leaving clinic at 11:30, HR=76, BP=113/26, R=18, O2sat=99. Patient left clinic at 1133. Neurology referral placed.
