Patient had adverse reaction to vaccine; rash on forehead. 9-month-old patient came to immunization department at 1630 on 6/22/26 for Pediarix and PCV after seeing PCP for well child check. Patient has received the same vaccines in the past and parents stated no adverse reaction noted the past. Patient received vaccine and was instructed to sit in the waiting room for 15 minutes per protocol. After ten minutes, i checked in with parents. Parents stated that the patient had newly formed redness on forehead. No SOB, swelling, spreading or lethargy note. LVN informed RN and PCP, both determined after seeing patient she should wait another 15 minutes. After Patient waited, symptoms improved. RN/ MD approved patient to leave with ED protocol.
