Symptom narrative

Patient presented for evaluation of a rash which developed 4 days ago. Rash was initially itchy, flat rash evolving into bumps. On 6/17, rash spreading, with one lesion now more widespread. No signs of pain or fever, normal appetite. No medications administered for the rash. Child observed scratching affected area. Skin examination: multiple lesions on left hip - small vesicles some are grouped - possible tenderness. Some lesions fluid filled some open and crusting - erythematous base, Lesions: Itchy and spreading - also tender.
