3:25 pm, Wiped area with alcohol pad, administered vaccine, intramuscular, site LD, using aseptic technique. 3:28 pm, pt stated " It feels really hot in here" proceeded to faint. Laid pt on floor, with knees lifted, came to in seconds, pale, diaphoretic, woozy, in and out at times, assessed breathing, normal, pulse 72, BP 94/61, O2 97 %. No nausea present. Applied cool cloth to head. 3:35 pm re-assed alert and talking, breathing, normal, pulse 75, BP 117/78, O2 97%, diaphoretic and pale. Applied another cool cloth on back of neck, gave water, and took a few sips. Also brought in a fan, in attempt to cool her down. 3:38 pm Husband and I placed pt back in chair, continued fanning, along with cool cloths on forehead and neck. Pt stated," I feel like I'm going to faint again" Husband and I placed pt sitting back on floor. Continued to monitor, pt drank more water, had her take deep breaths, continued air. I asked pt if they would like for me to call an ambulance, stated ?No this is normal for me, it happens every time I receive shots or blood draws?. Declined ambulance services. 3:40 pm Re-assed, alert and talking, BP 120/80, pulse 75, O2 97%, breathing normal. Color returned, along with decreased diaphoresis. Continued air, along with cool cloths on neck and forehead. Sips of water taken. 3:55 assessed vitals, BP 122/81, p 76, O2 97%, normal breathing, face pink in color, no diaphoresis or dizziness present. ?Stated I?m feeling a lot better?. Husband and I walked patient around room X 3, normal gait without difficulties. Instructed pt, if experiences an intensifying headache, nausea, vomiting, change in vision, sensitivity to light, mental status changes to go to the ER. I also suggested they could follow up with their PCP. Informed pt to give us a call with any questions or concerns.
