one vial exploded or erupted in hand /vaccine broke in hand while drawing up dose; one vial exploded or erupted in hand /vaccine broke in hand while drawing up dose; This is a spontaneous report received from an Other HCP. A patient (age and gender not provided) received pneumococcal 20-valent conjugate vaccine (diphtheria CRM197 protein) (PREVNAR 20), (Batch/Lot number: unknown) for immunisation; meningococcal group b Rlp2086 (TRUMENBA), (Batch/Lot number: unknown) for immunisation. The patient's relevant medical history and concomitant medications were not reported. The following information was reported: ACCIDENTAL EXPOSURE TO PRODUCT (non-serious), PRODUCT CONTAINER ISSUE (non-serious) and all described as "one vial exploded or erupted in hand /vaccine broke in hand while drawing up dose". Additional information: caller discussed discrepancy with Prevnar 20. In midst of using the Prevnar 20, one vial exploded or erupted in hand and wants to see how to go about that discrepancy with the one vial of Prevnar 20. Caller did not say what product this issue occurred with, caller can get that information if needed. Caller just sent email over the weekend saying a vaccine broke in hand while drawing up the dose and wants to know what do from here to report the loss and get replacement dose. Caller has vaccine name, lot, expiration, and everything to provide but caller does not have that information. Needs to know what to do to get replaced or reimbursed. Confirmed caller doesn't know the name of the product the issue occurred with, office uses Trumenba and Prevnar 20, so it was one of those, but caller was not sure which, could email to find out. Causality for "one vial exploded or erupted in hand /vaccine broke in hand while drawing up dose" was determined associated to device constituent of meningococcal group b Rlp2086 and pneumococcal 20-val conj vac (dipht CRM197 protein) (malfunction).
