Symptom narrative

Site: Pain at Injection Site-Mild, Site: Shoulder Joint Injury (prolonged pain, tingling, etc.)-Mild, Additional Details: Pt came to pharmacy on 6/25/26 stating he was having prolonged arm pain from flu shot received 1/2/26. He states he received it in his right arm, but our documentation states left arm. He states he came in 3 weeks after receiving vaccine stating it was still sore, but a colleague advised him that's normal. He states he's coming back in now since it's still sore and he likely needs surgery. He states you can see where the needle went in which was on the side of his arm, and not in the muscle.
