Symptom narrative

Administered 3 vaccines at 11:03 am. Pt. stated she felt dizzy/faint after 3rd vaccine (Hep A). Layed pt down supine and elevated legs. Pt. unresponsive x15 seconds. Recovered quickly and color returned to her cheeks/lips. Recovered by 11:05 am. Gave pt. juice and kept her laying supine while I administered 4th (and last vaccine--HPV #3). Took vital signs at 11:10 am and stable. Kept pt. laying down until completion of Quantiferon Gold test and she reported feeling normal. Did position changes slowly (laying, sitting, standing) and pt. denied any dizziness/lightheadedness. Stated she felt fine to drive and well upon discharge at 11:31 am. Pt. reports having had water this morning but no food.
