Symptom narrative

: During transport of vaccine inventory between health facilities on 4/7/26 and 4/17/26, there was a chance that the vaccines may not have been kept at a consistent temperature. All vaccines were packaged appropriately in two coolers and transported in an air conditioned van. Data loggers were used and no alarms were noted during the transport. Vaccines were unloaded and placed in the appropriate storage. The data loggers were kept with the storage materials but the temperature data were not uploaded. Once the issue of the missing data was recognized on 5/29/26, the vaccines were quarantined due to possible temperature excursion. Manufacturers for each affected were contacted, who provided vaccine stability data to inform decision making following temperature excursions. There is no indication that vaccines were exposed to temperatures out of their appropriate range at any time during the transport (cumulative time 90 minutes). However, since there is no documentation to demonstrate the temperatures remained in the appropriate range during transport, out of an abundance of caution, staff have contacted affected patients to recommend re-administration of the vaccine per CDC guidelines.
