LOT mix up; Mixed up the powder and diluent of different lot of Priorix and was administered to a patient; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a patient who received MMR (Priorix) solution for injection in pre-filled syringe (batch number 4FT9T5 with expiry date 01-MAR-2027 and batch number FA4C9 with expiry date 01-MAR-2027) for prophylaxis. On 22-JUN-2026, the patient received Priorix. On 22-JUN-2026, an unknown time after receiving Priorix, the patient experienced inappropriate preparation of medication (Verbatim: LOT mix up) and wrong technique in drug usage process (Verbatim: Mixed up the powder and diluent of different lot of Priorix and was administered to a patient). The outcome of the inappropriate preparation of medication and wrong technique in drug usage process were not applicable. Somebody mixed up the powder and diluent of different lot of Priorix and was administered to a patient. The reporter consented to follow up.
