Symptom narrative

Pt was to receive second dose of COVID vaccination at todays visit. COVID vaccination was administered and had BUD date of 07/15/26, however, upon further investigation when entering lot # into charting system, BUD date was entered as expiration date in our EHR, but the actual expiration date on vaccine vial/box was 06/18/26. Pt received expired COVID vaccination. No adverse reactions of note at this time.
