Client requested Japanese Encephalitis vaccine dose 1 for upcoming travel. Per vaccination record on shots client had Typhoid Vi, Hep A dose 1, and TDAP administered the day before at pharmacy and client denied any reactions. Per client request, Ixiaro dose 1 was administered. Approximately 2 minutes after vaccine administration, the client reported trouble breathing, stating, "feeling like throat closing." The client appeared flushed and diaphoretic, reported dizziness, and initial vital signs were HR 44, BP 90/50, RR 20, and SpO2 97%. EMS (911) was amended immediately. Epinephrine 0.5 mL IM was administered to the right deltoid. Following epinephrine administration, the client reported feeling better, stating, "just shaky." Repeat vital signs improved to HR 60, BP 92/54, RR 18, and SpO2 98%. The client remained under continuous nursing observation with serial assessments and vital signs obtained while awaiting EMS arrival. EMS arrived and evaluated the client. EMS reported that the client was presenting well following treatment and explained that transport to the emergency department was available but, given her clinical improvement, it was the client's decision whether to be transported. The client declined transport to the emergency department and signed the EMS refusal of transport form. The client reported feeling "much better ". Prior to leaving the clinic, the client was alert and oriented ×4. vital signs were HR 61, RR 16, BP 94/57, and SpO2 99%. The client denied shortness of breath, dizziness, throat tightness, or any other symptoms and ambulated from the clinic independently with a steady gait. Precautions were reviewed, and the client was instructed to seek immediate emergency medical care or call 911 if symptoms recurred, including shortness of breath, throat tightness, swelling, rash, or dizziness and the client verbalized understanding.
