Penmenvy - Only diluent administered; Penmenvy - Only diluent administered; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 16-year-old female patient who received Men ABCWY NVS (Penmenvy) suspension for injection in pre-filled syringe (batch number 7J5LF/A and batch number 7J5LF/B) for prophylaxis. On 04-JUN-2026, the patient received Penmenvy. On 04-JUN-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Penmenvy - Only diluent administered) and inappropriate dose of vaccine administered (Verbatim: Penmenvy - Only diluent administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. They were doing inventory and noticed that they had an extra vial of Penmenvy left, one of their last 3 patients who received Penmenvy could have received an incomplete dose. They were looking for guidance over this situation.
