Symptom narrative

Was the dose administered? Yes, but it was an incomplete dose; she no longer trusts the Merck STERILE DILUENT prefilled syringes; the luer lock became disconnected from the syringe /needle hub went off while administering vaccine. Vaccine spurted out.; luer lock became disconnected from the syringe while the needle was in the patient's arm/ The luer lock was loose. The incident occurred very quickly, and the needle was still in the patient at the time; luer lock on the prefilled syringes is sometimes loose and comes detached from the rest of the syringe causing some diluent/product to leak out/needle hub went off while administering vaccine. Vaccine spurted out.; she has started to use a different syringe to actually administer the product.; No additional AE; This Spontaneous report was received from Nurse and referred to a 1-year-old (Race Unknown ) ( Ethnicity Unknown) Female Infant patient. The patient's medical history, past medications and concomitant medications were not reported. On an unknown date, patient started therapy with sterile diluent Solution for injection 64466651:4175 (Batch/Lot Number: Z002901) (Expiration date: 25-Nov-2027) dose form was Solution for injection. On an unknown date, patient started therapy with non-product or non-merck product capsule Asked But Unknown Batch/Lot Number. On an unknown date, patient started therapy with sterile diluent Solution for injection 64466651:4175 Asked But Unknown Batch/Lot Number dose form was Solution for injection. On an unknown date, patient started therapy with measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live powder and solvent for solution for injection (Batch/Lot Number: Z006679) (Expiration date: 28-Mar-2027) dose form was Powder for solution for injection (Anatomical Site:Left Arm).
