Medication error/ supposed to be vaccinated with Tdap vaccine, not DTaP; This non-serious case was reported by a consumer via call center representative and described the occurrence of wrong vaccine administered in a 12-year-old female patient who received DTPa (Infanrix) suspension for injection in pre-filled syringe (batch number jp227 with expiry date 22-AUG-2027) for prophylaxis. Co-suspect products included DTPa (Reduced antigen) (Tdap Vaccine) solution for injection for prophylaxis. On 17-JUN-2026, the patient received Infanrix. The patient did not receive Tdap Vaccine. On 17-JUN-2026, an unknown time after receiving Infanrix, the patient experienced wrong vaccine administered (Verbatim: Medication error/ supposed to be vaccinated with Tdap vaccine, not DTaP). The outcome of the wrong vaccine administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 30-JUN-2026 The reporter was the mother of the consumer (patient). The reporter stated the consumer was vaccinated with Infanrix (Diphtheria and Tetanus Toxoids and Acellular Pertussis Vaccine Adsorbed) injectable suspension, DTaP. The reporter stated the consumer was supposed to be vaccinated with Tdap vaccine, not DTaP (medication error.). No other products were received by the patient.
