Symptom narrative

Patient always has pain in shoulders. States she unusually had 0 pain with either Heplisav shot when usually any vaccine causes severe pain. Says around a week after the 2nd shot she developed tingling along the inner side of her arm radiating down to her finger tips. No other signs or or symptoms but pain is moderate becoming severe at times. No reaction at injection site on dose 1 or 2

Current illness

Always has shoulder pain, nerve pain in back that she treats with Gabapentin

Medical history

Chronic back pain, Chronic shoulder pain

Other medications

Gabapentin

Allergies

Allergic to some psyciatric drugs but she cant remember their names

Lab data

Plans to see her PCP, (name withheld), for a normal physical in the next week. She states she beleives its July 10 or 15 the appointment. Will mention to the Dr then to see if he thinks it could be vaccine related

Prior vaccinations

Says always gets serious arm pain with vaccines but didnt experience any pain with either Heplisav vaccine
