Symptom narrative

Patient had just received a vaccination of the second HPV shot. Patient passed out in parking lot on the way to the vehicle with parents. He was brought back into lobby by parents. Front desk alerted me as triage nurse. I went to lobby and PT was very pale, sweaty, in a state of almost unconsciousness. I alerted provider, we would need EMS as I got a portable vital unit. I checked the patient?s vitals. His pulse was 45, oxygen was 97 and his blood pressure was around 69/35. Coworker alerted Dr to come to lobby to check patient and called EMS. I continued to monitor vitals. Patient again appeared to pass out. His head fell back onto the chair. He did appear aroused after calling his name while father rubbed his, arm. I rubbed the sternum area. Pulse was improving to low 60?s, oxygen continued to at same rate, blood pressure was slightly improved to 70/35. Dr administered the EPI pen his mother had with her, usually carried for his allergy injections. After administered, patient had a brief 3-5 seconds of jerking. Within a few minutes he was aroused enough to say he felt nauseated and requested some cold water. Vitals were pulse 67, oxygen 98% and a blood pressure of 74/32. Dr requested a manual blood pressure. provider came out and did manual blood pressure which was 80/38. Patient was drinking water and more alert by the time EMS arrived at around 4:45pm. Dr gave report to EMS.

No other illness at time of vaccination or one month prior.

allergic disposition ? bilateral hyperopia of eyes ? normal weight in childhood
