Symptom narrative

Started as soreness and swelling of the upper arm the morning after the vaccine, approx two weeks later had continued pain and limited use of the arm, reported symptoms to PCP, she thought might be bursitis, pain worsened in upper arm, went to pain management doctor who thought it might be frozen shoulder, given steroid injection with no improvement, pain management doctor then prescribed physical therapy, went twice weekly for one month with some improvement but still don't have full range of motion in that arm

GERD; high blood pressure; fibromyalgia; diabetes; urinary incontinence; back pain

160 mg Valsartan; 40 mg Protonix; 2000 mg Metformin; 60 mg Duloxetine HCL; 150 mg Pregabalin; 15 mg Oxybutynin ER; 15 mg Buspirone HCL; 750 mg Methocarbamol; 90 mcg Albuterol hfa; 220 mg Aleve
