Pharmacy labeled covid vaccine with expiration date of 08/10/2026. The vial had an expiration date of 7/12/2026. Vaccine was administered by medical assistant on 7/13. LVN noticed the label and vial expiration dates on 7/14. LVN noticed that database alerted him that it was expired when he scanned the vaccine. He informed RN Supervisor who contacted pharmacy manager who confirmed that vaccine was expired and vaccine was labeled in error by pharmacy. Pharmacy team will remove expired vaccine and replace.
