Symptom narrative

Tachycardia and dyspnea- Patient says that one day after receiving her covid vaccine, she felt her heart rate increase dramatically. She could feel it so she lied down and tried to calm down. She said it lasted 4 days off and on, with little improvement until it went away on it's own. heart racing up to 120's at rest and dyspnea with exertion that has been worsened from her chronic DOE. She called her cardiologist and made an appointment to see them. Cardiology examined her on 3/31/26 (7 days after start of reaction) but by then all her tests were normal. Patient said her cardiologist recommended patient not to receive any more covid vaccines

Esophageal dysphagia, Osteoporosis, Fatty Liver, Hypertension, Anxiety, Mixed Hyperlipidemia, Osteoarthritis

Simvastatin 20mg tablet, Famotidine 40mg tablet, Losartan 50mg tablet, Spironolactone 25mg tablet, Estradiol 0.01% vaginal cream, Cetirizine 5mg tablet, Diclofenac 1% gel, Melatonin 5mg tablet, vitamin D3 5000 units capsules.

She completed a cardiac stress test on 4/2/26- no evidence of ischemia.
