Symptom narrative

Vaccines were administered during office visit with PCP Dr on 7/22/2026 by MA staff. Patient's message to PCP on 7/24/26 at 2:05 AM: I just wanted to let you know that about 12 hours after getting my shots, I started to get a rash. It was splotchy on my right arm, a solid rash on my left hand and wrist with swelling, my right thigh and down the back, my left thigh, and a small vertical strip on my stomach. It's slowly fading, but my left hand is still red and swollen. This RN called patient on 7/24/26 at 8:33 AM to check in on status. Patient reported rash is improving in all areas, no itching. Swelling in left hand wrist are also going down. Offered patient an office visit with PCP to further evaluate rash and swelling. She declined given symptoms are improving, will monitor over the weekend and call PCP's office if symptoms are still persistent or worsening.

Medical history

venous stasis dermatitis of lower extremities, stage 3a chronic kidney disease, high cholesterol, hypothyroidism, type 2 diabetes, restless leg, primary narcolepsy, major depression, generalized anxiety disorder, OCD

Other medications

Levothyroxine 100 mcg daily, Mirapex 0.25 mg at bedtime, Adderall 90-120 mg daily, Paroxetine 20 mg daily, pravastatin 10 g daily, hydrochlorothiazide 25 mg daily.

Allergies

Atorvastatin, Vortioxetine, adhesive tape
