Patient was administered a lower dose amount than needed for Covid-19 Spikevax 2025-2026 vaccine. Patient received .25ml instead of .5ml
Patient was give .5mL of Spikevax when she should have received .25mL. Dr. did call parent to notifiy her of this incident.

Muscle pain, nausea, headache, urge to vomit. Essentially, flu-like symptoms without the actual flu. I don?t know the name of the manufacturer because it?s in light pen on the carbon copy.
