Pt claimed that his left arm started hurting after receiving a vaccine. Pain on his arm got worse over time. It's been now over a month and pain is still there whenever he moves his arm.

there was no adverse reaction. The patient came in to get a hep a vaccine (havrix) and priorix (mmr) was mistakenly given. This was a breakdown in our safety system and why we are reporting.

Patient experiencing swollen lymph notes, bumps, rash to neck and underarm lymph nodes. Saw urgent care ~2-3 weeks after initial vaccine, and was told likely delayed reaction to vaccine, rash was resolving. No further treatment.
