Patient was given vaccine too high in arm resulting in pain and immobility issues. She went to provider who identified it was given too high in her arm and she was also given a steroid shot to help with the pain.
Patient is 14 years old but was given 25 mcg/0.25mL dose which is for ages 6 months - 11 years old.
