SWELLING AROUND SITE 2 INCHES, EDEMA TO SITE, HOT TO TOUCH OVER SITE AND ERYTHEMYTOUS. TENDER. BENEDRYL 25 MG TAB PO Q 6-8 HOURS PRN RASH, AND AUGMENTIN 875 MG TAB, 1 TAB PO BID 10 DAYS.
I gave this patient the high dose Flu vaccine. Patient should have had the regular dose Flu vaccine . Patient was made aware of this and also any side effects she might have. No adverse reactions.

altered mental status 2 days after vaccine; became more sleepy and lethargic throughout day; not able to speak full sentences
Right arm warm red 8 cm round area with bullous blistering around the edges.
Patient started itching all over and eyes started watering and closing. Administered benadryl by oral route and monitored patient. They stated they felt fine after about 15 mins and went home.
