Reconstitute was done incorrectly for Pentacel. Powder (HIB) was reconstituted with Sterile water instead of the provided reconstitute. No harm to patient. NO adverse effect. All other vaccines given correctly.

Patient came to pharmacy 9/17/25 and said her arm became red and itchy after her vaccine 9/13/25. She has been applying ice and is starting to apply hydrocortisone cream and taking benadryl by mouth.
Difficulty breathing > improved after administered 1 dose of epipen 0.3mg. Back spasms/pain. Ambulance took her away.
