Provider ordered Vaxelis plus Hep B vaccine and both vaccines were adminstered to the patient by LPN. . Second check was done by CMA. Vaxelis contains the Hep B vaccine so patient should not have received the Hep B vaccine along with the Vaxelis.
Patient was given Kinrix for dose #3 of IPV and #3 of Dtap. Patient was not recommended. 4-6 years old vaccine is not indicated for these doses in the series.
