Provider accidentally administered the Pfizer COVID Comirnaty 12yrs and older vaccine to the patient, who was 11 years old at time of vaccine administration.
Patient developed hives on her arms, thighs and trunk the following day after Tdap injection was given for post puncture injury. Patient is currently receiving chemotherapy treatments. She was seen at our local clinic for a steroid injection on 12/15/2025.
The patient was given Hep B peds/Adol and should have been given Hep B Adult. There was no adverse reaction and patient tolerated well.
Redness and warmth around the injection site. Erythema spread to patients upper arm. Itchiness noted around the injection site. SX worse 24 hours after injection. Patient seen by health care team 4 days later and sx have been improving.
