MMRV vaccine was administered instead of MMR. The manufacturing company was contacted to verify if patient needed to repeat vaccine due to the error.
ProQuad and MMR II given on the same day instead of MMRII and Varivax. Per parent the child was fine post vaccine. The child did not experience fever, illness, rash. vomiting or diarrhea. The parent states she slept a bit more than usual the first 24 hours.
