Shingrix vaccine was mixed with the wrong dilutant (used the MMR/Varicella vaccine dilutant). No adverse effects. A repeat Shingrix vaccine was given 3/30/26.
MMR and Varicella vaccine were ordered for this patient. Medical assistant inadvertently gave a Produd/ MMRV instead of the regular MMR vaccine. Pt also received Varicella vaccine separately.
