Vaccine expired 4/30/2026 and was erroneously administered to patient.
Patient was 28.5 weeks, first pregnancy when receiving the vaccine. Estimated date of delivery is 8/14/26. Patient currently weighs 142.6. Patient was given an adult dose of Hep B vaccine. Provider and patient was notified of the vaccine given.
