Patient developed localized allergic reaction- 18 cm x 10 cm area on left upper thigh of erythema, induration, warmth, pruritic, tender. Onset approximately 24 hours after vaccine administration.
She has a fever and also a circular welt on her arm at the injection site
The vaccine was administered outside the beyond use date. the beyond use date was 03/21/2026 and was administered today 03/25/2026
DTaP was given, however she was supposed to have been given a Tdap instead
redness and swelling and on the same side of the injection, 2 days after vaccine administration. tender to the touch. looks like a red mild possible rash, but doesn't indicate itching, but patient self described as "rash"
Exactly a week later she had a severe fever that was difficult to control. Sometimes exceeding 104 F. Urgent care couldn?t lower her temp without putting her on a Tylenol and Motrin plan (grouped 1 hour apart) at 6 hour intervals.

The injection was more painful than most. The site is still sore with limited arm mobility.
Initial symptoms were multiple. Including headaches, chest pain and difficulty breathing. Eventually patient died on October 4th 2024 from a heart attack.
