Pt was seen at Hospital on 11/20/2025 for seizure like activity. Had multiple test, see box 19 and was discharged. Returned to ER, with seizure like activity, testing done and transferred to another hospital.
Patient received COVID vaccination 11/3. Patient presented to ED 11/26 with neurological dysfunction, emesis, and headache; 11/27 multiple ischemic strokes were identified in the cerebellum, thalamus, and occipital regions.
The next morning following vaccine administration patient reports high fever, fatigue, lack of appetite, and body aches. Patient took acetaminophen to help relieve symptoms. By next day sypmtoms resolved without health care provider intervention
