Vaccine was given by "accident". Patient was not scheduled to recieve this vaccine as he had already recieved his 2 doses at his 2 and 4 month appointment. The MA gave this vaccine to our son without Doctor and Parents Permission/Knowledge.
Patient was given MMR and Varicella combo vaccine, Proquad, which is for patient's 12 years or younger. Patient did not show symptoms, tolerated well. No adverse reaction.
