Redness, itching, and swelling rash/hives on both upper arms on and around injection sites. Pt with no noted reparatory distress, SOB, or angioedema on exam. OTC antihistamine recommended and Oral Medrol Rx. ER precaution in place.
No adverse event. Patient was administered DTap when provider originally ordered Tdap. Incorrect vaccine was given. Provider spoke with patient today and patient reports doing well. Per provider does not need to return to clinic.
